Job summary

Habitat Health is seeking an Outreach Specialist in California to support growth of the PACE program through community engagement and strategic outreach. You will develop partnerships with providers and senior-serving organizations to generate referrals and guide participants from referral to enrollment.

You will manage enrollment communications, track activities in the CRM, and contribute to program improvements while representing Habitat Health at events and in-center settings.

Qualifications

  • Bachelor's degree or equivalent experience in a related field.

Responsibilities

  • Identify opportunities to expand referral sources and improve outreach using market insights and performance data.
  • Maintain accurate referral, outreach, and relationship activity in CRM and other systems.
  • Monitor outreach activities and performance metrics to drive continuous improvement.
  • Ensure outreach and marketing activities comply with policies and regulatory requirements governing PACE marketing.
  • Participate in evening and weekend events as needed.
  • Collaborate with Center leadership on retention strategies.
  • Perform other related duties as assigned.

Role Scope

The Outreach Specialist plays a key role in supporting Habitat Health's growth by building trusted relationships with healthcare providers, community organizations, referral partners, prospective participants, caregivers, and other stakeholders. This role increases awareness of Habitat Health's PACE program through community engagement, strategic outreach, and relationship management while partnering closely with the broader Outreach and Enrollment team to support a seamless participant journey from referral through enrollment.

Core Responsibilities & Expectations for the Role
  • Builds trusted relationships with healthcare providers, community organizations, senior living communities, post-acute providers, and other referral partners to generate qualified referrals and support enrollment growth.
  • Develops and executes outreach strategies that increase awareness of Habitat Health's PACE program and strengthen community partnerships.
  • Represents Habitat Health through community presentations, networking events, educational sessions, health fairs, and other outreach initiatives.
  • Drives the enrollment process as the case manager for prospective participants, including reviewing, processing, and updating applications and required documentation, distributing relevant enrollment materials, and managing communications.
  • Identifies opportunities to expand referral sources, strengthen community partnerships, and improve outreach strategies using market insights and performance data.
  • Maintains accurate referral, outreach, and relationship activity within CRM and other organizational systems in a timely and accurate manner.
  • Monitors outreach activities and performance metrics, using data to prioritize work and contribute to continuous improvement.
  • Ensures outreach and marketing activities comply with applicable organizational policies and federal and state regulatory requirements governing PACE marketing practices.
  • Participates in evening and weekend community events as needed to support organizational growth objectives.
  • Contributes to a program experience that Participants want to join, a team culture that cares and creates joy, and an environment where all participants and team members belong.
  • Conducts tele, home visits and in-center evaluations for the prospective participant for potential eligibility for care.
  • Collaborates with Center leadership to develop retention strategies.
  • Performs other related duties as assigned.
Required Qualifications
  • Bachelor's degree in Business, Marketing, Healthcare Administration, Public Health, or a related field, or an equivalent combination of education and/or relevant experience.
  • Minimum of one (1) year of experience in healthcare sales, business development, provider relations, community outreach, enrollment, or a similar relationship-focused role, preferably within a healthcare environment.
  • Demonstrated ability to develop and maintain trusted relationships with healthcare providers, community organizations, referral partners, and other external stakeholders to support organizational growth.
  • Experience identifying, cultivating, and managing referral sources, community partnerships, or business development opportunities that contribute to enrollment growth.
  • Experience working with Medicare, Medicaid, state and county eligibility programs, or other healthcare eligibility and referral processes preferred.
  • Experience working with older adults, frail elderly populations, and caregivers.
  • Strong organizational skills with the ability to manage multiple priorities, maintain accurate records, and ensure timely follow-through.
  • Strong communication, presentation, and collaboration skills, with the ability to effectively educate healthcare providers, community partners, prospective participants, caregivers, and cross-functional teams on the value of Habitat Health's PACE program.
  • Demonstrated ability to work independently while collaborating across teams to achieve shared outcomes.
  • Experience using Microsoft Office and the ability to quickly learn CRM, referral management, and participant relationship management systems.
  • Proof of a valid driver's license, reliable personal transportation, and automobile insurance that meets applicable state requirements, with the ability to travel throughout the service area.
  • Active CPR/BLS certification or the ability to obtain certification within 30 days of hire.
  • Meets all applicable participant-facing health requirements, including TB screening and required immunizations.

Compensation

The anticipated base salary range for this position is $77,000 - $81,000 annually. This position is also eligible for a target monthly bonus and may be eligible for additional incentive compensation, as applicable.

The final compensation offered will be determined based on factors including, but not limited to, the candidate's qualifications, skills, experience, education, certifications, geographical location, and business needs.

In addition, this position is eligible for a comprehensive benefits package, including medical, dental, and vision insurance; paid parental leave, short- and long-term disability coverage; life insurance; flexible spending accounts; a 401(k) savings plan that includes a match; paid time off; and company paid holidays.

Employment with Habitat Health is at-will.
