Role Scope : The Enrollment Specialist plays a key role in supporting Habitat Health's growth by overseeing and managing the end-to-end enrollment journey for prospective participants. This role partners closely with the Outreach Specialist, Enrollment RN, and Interdisciplinary Team (IDT) to guide prospective participants and caregivers through the enrollment process, remove barriers to enrollment, and support informed decision-making. By coordinating enrollment activities and fostering trusted relationships, the Enrollment Specialist deliversa seamless, participant-centered enrollment experience while ensuring compliance with applicable organizational policies and regulatory requirements.

Core Responsibilities & Expectations for the Role

  • Buildstrusted relationships with prospective participants, caregivers, to support enrollment growth and participant engagement.
  • Drivesthe enrollment process as the case manager for prospective participants, including reviewing, processing, and updating applications andrequireddocumentation, distributing relevant enrollment materials, and managing communications
  • Coordinates assigned enrollment cases by managing applications, documentation, participant communications, andrequiredfollow-up activities ina timelyandaccuratemanner.
  • Partners closely with the Outreach team, Enrollment RN, IDT, and center leaders to deliver a coordinated, participant-centered enrollment experience.
  • Conducts enrollment visitsinparticipants' homes, the center, orvirtually toassess needs, support eligibility activities, and advance the enrollment process.
  • Identifiesand helps resolve barriers to enrollment by proactively coordinating resources,maintainingcommunication, and facilitating a smooth transition into the PACE program.
  • Maintainsaccurateenrollmentdocumentation and participant recordsin accordance withorganizational policies and regulatory requirements.
  • Monitors enrollment activities and performance metrics, using data to prioritize work and contribute to continuous improvement.
  • Contributes to community awareness of Habitat Health byrepresentingthe organization professionally and supporting outreach and referral development activities.
  • Contribute to a program experience that Participants want to join, a team culture that cares and creates joy, and an environment where all participants and team members belong.
  • Performs other related duties as assigned.

Required Qualifications:

  • Bachelor's degree or equivalent combination of education and/orrelevant experience.
  • Minimum of one (1) year of experience in healthcare sales, enrollment, customer success, business development, care navigation, or a similar relationship-focused role, preferably within a healthcare environment.
  • Demonstrated ability to build trusted relationships, educate prospective participants, and guide individuals through complex decision-making with empathy and professionalism.
  • Strong organizational skills with the ability to manage multiple priorities whilemaintainingattention to detail and ensuringtimelyfollow-through.
  • Strong communicationand collaboration skills, with the ability to effectively engage prospective participants, caregivers, referral partners, and cross-functional teams to deliver seamless enrollment experience.
  • Demonstrated ability to work independently while collaborating across teams to achieve shared outcomes.
  • Experience using Microsoft Office and the ability to quickly learn CRM, enrollment management, and participant relationship management systems.
  • Proof of a valid driver's license, reliable personal transportation, and automobile insurance that meets applicable state requirements, with the ability to travel throughout the service area.
  • Active CPR/BLS certification or the ability to obtain certification within30 daysof hire.
  • Meets all applicable participant-facing health requirements, including TB screening and required immunizations.

Nice to have:

  • Experience supporting enrollment, outreach, participant engagement, or business development within PACE, Medicare, Medicaid, managed care, or other value-based healthcare environments.
  • Knowledge of PACE, CMS, and applicable federal and state regulatory requirements related to participant enrollment, eligibility, andcarecoordination.
  • Experience using CRM, enrollment management, or participant relationship management systems.
  • Bilingual: Spanish, Mandarin, or Cantonese.

Compensation:

The anticipated base salary range for this position starts at $70,340 annually . This position is also eligible for a target monthly bonus and may be eligible foradditionalincentive compensation, as applicable.

The final compensation offered will bedeterminedbased on factors including, but not limited to, the candidate's qualifications, skills, experience, education, certifications, geographical location, and business needs.

In addition, this position is eligible for a comprehensive benefits package, including medical, dental, and vision insurance;paid parental leave,short- and long-termdisability coverage; life insurance; flexible spending accounts; a 401(k) savings planthat includes a match; paid time off; and companypaid holidays.
