Job Title: Outreach Specialist - High-Risk Care Management Program (HRCM)

The Outreach Specialist for the High-Risk Care Management Program (HRCM) will engage with patients prior to discharge across Moses, Wakefield, and Weiler Campuses, with location varying based on need. The HRCM program aims to assist high-acuity patients in transitioning from inpatient to outpatient care. Responsibilities include aiding patients in complying with scheduled appointments, engaging with onsite and offsite psychiatric treatments, and performing home visits when necessary to foster engagement. The Outreach Specialist will also support initial patient engagement, outreach efforts, and accompany patients to appointments as needed. This program is part of the Behavioral Health Centers of Excellence award.

This is a 100% grant-funded position requiring travel to various locations based on departmental needs.

Requirements:

• High School Diploma/GED required; Bachelor’s degree preferred
• Certification from or provisional certification as a New York Certified Peer Specialist by an OMH-approved program
• Excellent communication skills (oral, written, interpersonal)
• Ability to find creative solutions to challenges
• Familiarity with Microsoft Word and Excel

Peer Specialists/Advocates with a credential from a recognized certifying authority (e.g., Certified Recovery Peer Advocate) are eligible to work in MHOTRS programs, provided they qualify for and obtain provisional OMH Peer Certification within 12 months of hire. They are expected to complete full OMH Peer Certification or credentialing after provisional certification.
