The Outreach Coordinator is responsible for coordinating and supporting community-based HIV, Hepatitis C (HCV), and Syphilis prevention, education, screening, and linkage-to-care activities. This position provides day-to-day guidance to outreach staff and helps ensure that all team members are properly trained, competent, and compliant with organizational, grant, and regulatory requirements.

The Outreach Coordinator works closely with program leadership, clinical teams, community partners, and outreach staff to expand access to testing and prevention services, particularly among communities disproportionately impacted by HIV, HCV, and sexually transmitted infections.

Job Responsibility

• Contacts patients; schedules appointments for consultations, agency meetings and/or diagnostic tests.
• Collects and organizes data required for statistical reports and completes reports, as required.
• Attends and participates in meetings as appropriate.
• Sets-up files and maintains records.
• Provides incoming and in-process orientation.
• Based on position, may be required to provide bi-lingual translation and/or guidance.
• Coordinate community outreach activities designed to increase awareness of HIV, HCV, Syphilis, PrEP, PEP, sexual health, and available prevention services.
• Develop and maintain relationships with community-based organizations, shelters, healthcare facilities, schools/colleges when appropriate, social service agencies, and other community partners.
• Identify opportunities for testing events and outreach activities in communities with increased need.
• Assist with planning health fairs, mobile testing events, educational presentations, and targeted outreach initiatives.
• Support culturally responsive and stigma-free approaches to sexual health education and testing. Performs related duties as required. Job Qualification

• High School Diploma or equivalent required. Associate's or Bachelor's degree in Public Health, Health Education, Human Services, Social Work, Community Health, or a related field preferred.
• 1-3 years of relevant experience, required.
• Experience working in HIV/STI prevention, community outreach, public health, sexual health, infectious diseases, or a related field strongly preferred.
• Ability to work effectively with diverse populations and provide services in a respectful, nonjudgmental, and stigma-free manner.
