Position Summary:

Under the immediate direction of the Transitional Care Operation Manager, the Outreach Services Clerk provides administrative support to the Street Outreach Team. This role manages the documentation of engagements that the street outreach team members conduct in the field, including assisting in billing and recording the use of grant-based funds to aid people experiencing homelessness. This role requires a high level of detail and organizational skills. This role is expected to contribute to a positive team culture with a high degree of professionalism and a person-centered, trauma informed regard for people experiencing mental health and substance use issues.

KNOWLEDGE, SKILLS, AND ABILITIES:
  • Use of Microsoft Office Suite, including Excel, Microsoft Word, PowerPoint, etc.
  • Ability to communicate effectively, both verbally and through written communication.
  • Organizational skills to manage the recording and allocation of resources to support street outreach initiatives.
  • Ability to maintain confidentiality in a healthcare environment.
  • Ability to prepare clear, concise, and accurate reporting for internal and external stakeholders and community partners.
JOB DUTIES AND RESPONSIBILITIES:
  • Assisting street outreach teams in documenting engagement and contacts with individuals in the field.
  • Purchasing, ordering, tracking, and documentation of grant-funded resources.
  • Maintaining a well-organized workspace to support outreach team job duties, including but not limited to: creating harm reduction packages, packing food items for distribution, creating supply packs for basic needs and living support.
  • Maintaining and recording documentation for harm reduction and testing supplies, including ordering needs, distribution to individuals and community partners.
  • Collect data and create reports of outreach data for presentations to external stakeholders and internal leadership.
  • Inspection of and reporting of maintenance needs for outreach vehicles.
  • Contacting local businesses and social services to identify and source supplies and consumables for outreach support.
  • Schedule and organize meetings and events for the Outreach team.
  • Performs other reasonably related duties as assigned by the immediate supervisor or other management as requested.
  • Meet or exceed “satisfactory” expectations at any scheduled job performance evaluation.
  • Regular and predictable attendance is essential and expected for this position.
EDUCATION, QUALIFICATIONS, AND EXPERIENCE:
  • High School Diploma or equivalent, required
  • Experience documenting in an electronic health record, preferred
  • Experience in purchasing and resource allocation, preferred
  • Certified Peer Recovery Support Specialist (CPRSS) Certification, preferred
WORK ENVIRONMENT:
  • This role will require occasional travel within the metropolitan area.
  • This role will primarily be office-based, working in a large, open space.
  • This role will not be primarily client-facing but may include interactions with individuals to direct to services on a large campus.
