Job Description

The Outreach Nurse supports the safe and efficient delivery of community-based vaccination and sexually transmitted infection (STI) services. Working closely with Community Health and Wellness partners, the nurse coordinates and monitors outreach activities and related clinical functions while exercising independent professional judgment.

The Outreach Nurse ensures patient safety at outreach events and helps expand NATIVE HEALTH’s mobile, school-based, and community-based vaccination and STI services for underserved and minority populations throughout Maricopa County. The position also participates in program monitoring and ongoing professional education to remain current on vaccine and STI-related clinical practices and advancements. The Outreach Nurse Reports administratively to the Health Services Director and works under the clinical oversight of the Chief Medical Officer or designated nursing leader.

KEY ACCOUNTABILITIES:

1. Monitors all of the clinical operations and related functions of outreach events.

Measures of Success:

  • Maintains current knowledge of vaccine recommendations and STI screening, testing, and treatment guidelines.
  • Determines the vaccines, STI testing supplies, emergency equipment, and other clinical materials required for each outreach event.
  • Ensures the timely and accurate documentation of vaccine distribution and retrieval, testing supplies, inventory, and required clinical logs.
  • Prepares or oversees the preparation and administration of vaccines in accordance with organizational policies, clinical protocols, standing orders, and applicable scope-of-practice requirements.
  • Minimizes vaccine wastage through effective planning and consistent communication with the registration and outreach teams regarding anticipated attendance.
  • Conducts STI screening and testing in outreach settings and coordinates timely follow-up for individuals with positive results, including notification, education, referral, and linkage to care.
  • Reviews vaccine screening questionnaires with patients to confirm eligibility and identify potential contraindications or precautions before administration.
  • Ensures required emergency medical equipment and supplies are available at each event and routinely inspected for readiness.
  • Provides timely and appropriate responses to patient questions, concerns, and clinical needs.
  • Collaborate with outreach staff to support efficient event operations, including scheduling, preregistration, clinical readiness, logistics, mobile-unit preparation, deployment, breakdown, and post-event follow-up.
  • Directs patient and community inquiries regarding vaccines and STI screening, testing, and treatment to the appropriate internal staff or services.
  • Monitors vaccine recipients during required observation periods and promptly provides assessment and intervention when needed to protect patient safety.
  • Reports vaccine wastage in accordance with Vaccines for Children (VFC) program requirements and organizational procedures.
  • Participates in culturally responsive and trauma-informed outreach activities that promote health equity and improve access to preventive services for underserved and minority populations.

2. Coordinates the immunization and outreach-based STI testing programs associated with community outreach events.

Measures of Success:

  • Coordinates immunization and outreach-based STI screening and testing services to support safe and efficient event operations.
  • Supports the development of program baselines, performance monitoring, key performance indicator tracking, grant reporting, and quality-improvement initiatives aligned with program goals. Measures may include outreach volume, immunization uptake, STI screening and testing rates, prevention education, and linkage-to-care outcomes.
  • Tracks program activities and outcomes, including mobile vaccination events, school-based clinics, culturally tailored outreach activities, individuals reached, vaccines administered, individuals screened or tested, positive results requiring follow-up, and successful linkage to care.
  • Orders vaccines, testing materials, and outreach supplies in a timely manner and maintains effective inventory controls.
  • Maintains accurate inventory logs, routinely monitors supply levels, and proactively reorders supplies as needed.
  • Maintains accountability for temperature-monitoring devices, including digital data loggers, and confirms that required temperature readings are within acceptable ranges before vaccines are placed in refrigerated or frozen storage.
  • Accurately documents vaccine administration in the electronic health record and the Arizona State Immunization Information System (ASIIS) and completes all required reporting and data entry.
  • Reports vaccine-related adverse events in accordance with Vaccine Adverse Event Reporting System (VAERS) requirements and organizational procedures.
  • Supports implementation of the Shield for Life vaccination program and monitors progress toward established grant objectives, including increasing vaccination coverage among uninsured and underinsured Maricopa County residents aged six months and older by 25%; conducting at least 48 mobile vaccination events, 12 school-based vaccination clinics, and 12 culturally tailored outreach campaigns; and reaching at least 10,000 individuals with recommended vaccines by December 30, 2027.
  • Tracks and reports childhood, adult, and influenza immunization performance measures; individuals reached; vaccines administered; event type and location; patient demographics; educational materials distributed; and other outcomes required by NATIVE HEALTH, the funding organization, or applicable regulatory agencies. Tracks program outcomes toward goal progress.
  • Supports culturally responsive vaccine education and hesitancy-reduction activities in partnership with schools, community-based and faith-based organizations, senior-serving organizations, community leaders, and other outreach partners.
