Job summary

NATIVE HEALTH in Phoenix, AZ seeks an Outreach Nurse to support the safe, efficient delivery of community vaccination and STI services across mobile, school-based, and community sites. You will coordinate outreach activities and perform clinical functions with independent judgment.

You will administer vaccines, conduct STI testing, document in EMR and ASIIS, and ensure patient safety while advancing health equity for underserved populations in Maricopa County.

Qualifications

• Graduate of accredited nursing program.
• Current state RN or LPN licensure in Arizona.
• Minimum of 3 years nursing experience.
• Flexible schedule including weekends/evenings; travel across Maricopa County.
• Ability to work outdoors and in non-traditional settings.
• Maintains professional demeanor and high ethical standards.

Responsibilities

• Provide nursing care and immunization services per scope of practice.
• Administer vaccines and STI testing at outreach events.
• Document vaccinations and testing in electronic health records and state registries.
• Monitor patients during observation periods and provide needed interventions.
• Coordinate logistics for outreach events including scheduling, preregistration, and follow-up.
• Educate patients on vaccines and STI prevention; refer to care as appropriate.
