Care Resource Community Health Centers, Inc. is seeking an Outreach and Housing Navigator to identify and assist families facing homelessness. You will conduct intake assessments, provide individualized housing navigation, and coordinate referrals to rapid rehousing and supportive housing programs.
The role emphasizes trauma‑informed, culturally responsive service delivery, strong collaboration with landlords and community partners, and meticulous documentation across systems.

Ensures proper handwashing according to the Centers for Disease Control and Prevention guidelines.

This work requires the following physical activities: constant sitting, walking, and hearing, talking in person and talking on the phone. Occasional driving stretching/reaching, and standing are required. Work is usually performed in an office setting and in the field conducting outreach.
Reliable personal transportation is required to conduct field‑based outreach and client support activities.
Regular travel to client homes, shelters, hotels, community locations, and public spaces is expected as part of outreach and housing navigation duties.
Occasional travel to airports or other designated locations may be required to support family reunification or placement needs.
High School Diploma is required.
The candidate should have demonstrated experience supporting families experiencing homelessness, with a strong understanding of the challenges they face and the community resources and financial assistance programs that promote housing stability. Able to work with diverse, multicultural populations while providing family-centered, trauma-informed services. The candidate should also have experience with key housing and homelessness programs—including EHE, HOPWA, Rapid Re‑Housing, Permanent Supportive Housing, and Federally Qualified Health Center (FQHC) programs to effectively support families in securing and maintaining long‑term housing stability.
