Southern Jersey Family Medical Centers, Inc. seeks an Entry Level Agricultural Health Educator to coordinate care for agricultural workers, including referrals, medication assistance, advocacy, and education. The role covers interpretation, transportation, and outreach to ensure timely access to services.

You will liaise with providers, facilitate appointments, and support patients across the care continuum while maintaining high professional standards.

Qualifications

• Bi-Lingual in Spanish and/or Creole.
• Experience in case management and outreach.

• Strong communication skills with customers.

Responsibilities

• Manage cases of agricultural worker patients needing additional care including specialist and diagnostic appointments.
• Receive information and communicate with providers on patient status and arrange follow-ups.
• Advocate and negotiate for patients to obtain needed services.
• Serve as liaison between specialist providers and agricultural workers.
• Organize services to ensure timely access for clients.
• Provide health education based on medical conditions.
• Make periodic follow-up calls to patients.
• Interpret for doctor/dental visits as needed.
• Provide or arrange transportation for workers to community resources.
• Perform tasks assigned by Manager or Coordinator.

Southern Jersey Family Medical Centers, Inc. is looking for anAgricultural Health Educator who will embrace our mission to removebarriers to care for patients within the Communities we serve.

Summary: The Outreach Health Educator coordinatescare for our Agricultural Workers to ensure that they receive theservices needed after their initial visit through Southern JerseyFamily Medical Centers, Inc. This includes referrals for care andspecialist services, assistance with medication and diagnostic exams,advocacy for the patient, treatment planning and patient education.The Outreach Health Educator also provides other services toAgricultural workers, including but not limited to interpretation,transportation, and outreach.

Hours:Full-Time (8am - 5pm) Monday - Friday - Someweekends may be required to aid in participation of community events.

Position Responsibilities/Duties:

• * Manage cases of agricultural worker patients who need additionalcare including specialist appointments, diagnostic appointments, andCharity Care appointments.


• * Receive information and communicatewith providers on patient’s status and make necessary appointmentsand/or follow up with that patient.


• * Advocate and negotiate forpatients to obtain services that fit the needs of that patient.


• * Serve as a liaison between the specialist providers andthe Agricultural workers


• * Organize to ensure clients get theservices they need in a timely fashion.


• * Provide healtheducation information for patients based on their medical condition.


• * Make periodic phone calls to patient to follow up.


• *Maintain positive relationships with referring community resources/health facilities and provides appropriate referral to health centerand other community resources.


• * Interpret for doctor/dentalvisits for patients.


• * Answer phone calls and make appointmentsfor Southern Jersey Family Medical Centers.


• * Provide or arrangetransportation as needed to/for Agricultural workers needingassistance getting to community organizations and healthcenter.


• * Embody the highest standards of professional thinking,bearing, conduct and appearance.


• * Perform other tasks asassigned by Manager or Coordinator.

Experience and Skills:
