A typical day for an Outreach Worker may include:

Meeting patients in a clinic, hospital, ER, and where patients are located and conducting needs assessment using Social Determinants of Health Tool.
Scheduling weekly follow-up calls and home visits to patients.
Helping patients with social issues like homelessness, substance abuse, hunger, transportation, and many other social issues that are impacting the members lives.
Assisting patients with organizing their records, making follow-up appointments with Primary Care Physicians or Specialists.
Helping patients with the tasks of getting medical care, and working on health goals by helping to plan healthy meals or finding time to exercise.
Helping patients access resources, including quality of care, childcare services, food, housing, transportation.
Maintaining relationships with Community Based Organizations.
Traveling and meeting members where they are at, which may include local Churches, homeless shelters, and members' residences.
Pursuing continued professional development through training, education, and organizational resources that support long-term career growth.

To be considered for this Outreach Community Health Worker opportunity

You are passionate about supporting individuals in the community while helping them overcome barriers to meet their health goals.
CHW will have a bachelors or equivalent level of education preferred, training in a related social service or human service field, and experience working in the community and with patients with co-morbid conditions
Formal Community Health Worker Certification a definite plus
Candidate must have the ability to demonstrate the delivery of culturally sensitive services to reduce stigma, ability to provide trauma-informed care and be able to function effectively within a team-based, inter-professional approach,
A valid drivers license is required for this position and employees will be required to conduct outreach and support services outside of traditional work hours, such as evenings/weekends
