Job summary

SierraVista is seeking an Outpatient Therapist (LPC/LCSW/LMFT) in Johnstown, CO to deliver telehealth and in-person care across diverse patient populations. You will provide individual, group, and family therapy using CBT/DBT/trauma-informed approaches, complete intake/diagnostic assessments, craft treatment plans, and maintain timely progress notes.

You will coordinate care with PCPs, schools, courts, and community supports to ensure continuity of care.

Qualifications

• Master’s degree with state license (LPCC, LCSW, LISW, LMFT).
• Experience with SUD, co‑occurring, or severe mental illness preferred.
• Strong telehealth etiquette and documentation skills.

Responsibilities

• Provide individual, group, and family therapy using CBT/DBT/trauma‑informed approaches.
• Complete intake/diagnostic assessments, treatment plans, and measurable progress notes.
• Coordinate care with PCPs, schools, courts, and community supports as appropriate.
