West Oaks Outpatient Branches in Houston seeks a dedicated Outpatient Therapist to work with adults and adolescents across various care levels. The therapist will coordinate with patients, families, and multidisciplinary teams to plan treatment and discharge, while pursuing ongoing professional development.

Travel between Houston and Katy clinics may be required. Responsibilities include conducting psychosocial assessments, individual, group and family therapies, and collaborating with care

Qualifications

• Education required: Master’s Degree in Human Services related field.
• Preferred: Master’s in Social Work.

Responsibilities

• Complete psychosocial assessments, treatment plan reviews, individual, group and family therapy sessions within established timeframes, face-to-face or telephonically.
• Collaborate with multidisciplinary team in crisis intervention, create behavioral plans, case conferences and discharge planning.
• Conduct assigned therapeutic groups and provide education and advocacy to assist patients and families in accessing follow-up services.

West Oaks Outpatient Branches Houston is seeking a dedicated and positive Therapist to join our team of compassionate, dedicated professionals. The Outpatient Therapist will work with adults and adolescents who are experiencing a range of psychological and complex life experiences. The Therapist will also be responsible for collaborating with the patient, family/support, multidisciplinary team, outside providers, and community agencies to support and coordinate treatment and discharge planning. Outpatient Therapist will be responsible for continuing their personal development to enhance practice. Works with adolescent and children.

Job Duties/Responsibilities:

• Complete Psychosocial Assessments, treatment Plan reviews, individual therapy, group therapy and family therapy sessions within established timeframes either face to face or telephonically-with all patients, family and significant others.
• Collaborates with multi-disciplinary team including physicians, RNs, Care Managers and other healthcare team members in crisis intervention; creating behavioral plans, case conferences, client centered interventions and discharge planning.
• Conducts assigned therapeutic groups. Provides comprehensive education, instructions, advocacy information designed to assist the patient and significant others to identify and secure needed health care follow-up services.

Qualifications

Requirements:

• Education Required: Master’s Degree in Human Services related field. Preferred: Master’s in Social Work.
• Licensure: LPC, LPC-A, LMFT, or LMSW with LCDC or LCDC-I . CPR and hospital de-escalation certifications.
