Job summary

• Assess high‑risk patients and develop short‑term treatment plans.
• Lead patient and family meetings regarding care decisions and discharge planning.
• Advocate for vulnerable populations and connect families with community resources.
• Participate in performance improvement initiatives and care coordination activities.
• Support transitions along the continuum of care with timely processes.
• Deliver mental and behavioral health services and evidence‑based assessments.
• Maintain partnerships with community organizations and service agencies.
