Tampa General Hospital is seeking a Social Worker in the Emergency Department to support patients and families by managing psychosocial and economic co-morbidities. The role focuses on transitions of care, resource mobilization, and collaboration with the care team to optimize treatment outcomes.

The Social Worker will provide crisis intervention, facilitate discharge planning, and liaise with community resources to address barriers to care.

Competenze

• MSW degree from CSWE-accredited program.
• LCSW licensure required.
• Minimum 2 years of experience in a health care facility or program.

Mansioni

• Coordinate transitions of care for patients with high psychosocial needs.
• Collaborate with Care Manager and the interdisciplinary team to review multidisciplinary care plans.
• Provide crisis intervention and psychosocial assessments as part of the care team.
• Assist in safeguarding patients through social work interventions including guardianship, legal resources, and community referrals.
• Participate in multidisciplinary rounds and care coordination activities.
