This role involves managing complex psychosocial issues, coordinating care transitions, and providing therapeutic interventions to patients and families. Successful candidates will hold an MSW degree and preferably have clinical licensure. Full-time schedule from Monday to Friday with a focus on improving patient outcomes through effective communication and crisis management.

Qualifications

• Experience in a health care facility or program.
• Registered Clinical Social Worker Intern (RCSWI) required within 6 months.
• Licensed Clinical Social Worker (LCSW) preferred.

Responsibilities

• Create optimal outcomes by managing complex psychosocial co-morbidities.
• Coordinate transitions of care for patients with high psychosocial needs.
• Conduct family meetings for important patient decisions.
• Provide intervention for end-of-life, guardianship, and adoption.
• Perform psycho-social assessments for high-risk patients.
