Job summary

Trinity Health IHA Medical Group is seeking a compassionate Registered Nurse (RN) to join our outpatient care team in Michigan. You will perform clinical triage, care coordination, and patient education to improve outcomes and patient experience.

Responsibilities include assessing needs, coordinating care, managing referrals and authorizations, and supporting transitions of care while collaborating with physicians, care managers, and other health professionals.

Qualifications

• Graduate of an accredited Registered Nurse program.
• Valid, unrestricted RN license in Michigan.
• Valid CPR certification.
• 1-2 years of clinical experience in a medical/physician office preferred.

Responsibilities

• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance and patient education.
• Coordinate care and support transitions of care.
• Collaborate with providers, care managers, pharmacists, behavioral health specialists, and other partners.
• Manage medication refills, test results, referrals, and prior authorizations.
• Identify preventive care needs and support chronic disease management.
• Help patients access community resources and navigate barriers to care.
• Serve as a clinical resource and leader within the care team.
