• BSN from an accredited nursing program; if ADN then BSN within 5 years.

Responsibilities

• Performs essential duties in clinic/non-acute operations: full intakes, reconciliation of meds, care coordination, patient triage.
• Provides safe, holistic care in physician practice/outpatient setting, educating patients and families to achieve outcomes.
• Uses data to improve care and guide practice; serves as resource, preceptor and mentor to team.
• Ensures efficiency, prioritizes immediate needs and collaborates with team to maximize productivity.
