Job summary

Trinity Health IHA Medical Group in Michigan Chelsea is seeking a compassionate Registered Nurse to join our outpatient care team on a part-time, day-shift schedule. In this role you will triage by phone and in person, provide clinical guidance, educate patients, and coordinate care across providers and care managers.

You will support transitions of care, manage referrals and prior authorizations, and help patients access community resources while advancing care quality and patient experience.

Qualifications

• Graduate of an accredited Registered Nurse program.
• Active, unrestricted RN license in Michigan with CPR certification.
• 1–2 years of clinical experience in a medical office or physician practice preferred.

Responsibilities

• Assess and triage patient needs by phone and during in-office visits.
• Provide clinical guidance, patient education, and care coordination.
• Support transitions of care and post-discharge follow-up.
• Collaborate with providers, care managers, pharmacists, behavioral health specialists, and other partners.
• Manage medication refills, test results, referrals, and prior authorizations.
• Identify preventive care needs and support chronic disease management.
• Help patients access community resources and navigate barriers to care.
• Serve as a clinical resource and leader within the care team.
