Responsibilities

• Performs PT evaluation, reassessment and consultation as referred by a physician
• Develops plan of care, goals and discharge planning
• Educates patient and family on therapy goals and interventions
• Provides PT interventions within APTA guidelines and regulatory standards
• Participates in interdisciplinary discharge planning and care continuum
• Ensures patient safety, rights, confidentiality and compliance with regulations
• Identifies opportunities for performance improvement
• Advocates for the patient and maintains excellent rapport
• Delivers efficient, high-quality patient care services
• Attends mandated meetings and continuing education
• Communicates with physicians about progress and changes
• Adheres to hospital mission and departmental policies
