Job summary

Norwegian American Hospital in Chicago is seeking a Physical Therapist for outpatient registry services on a per diem basis. You will perform PT evaluations, establish plans of care, and contribute to patient progress under APTA guidelines.

This on-site role requires licensure in Illinois, clinical experience, and the ability to collaborate with a multidisciplinary team; bilingual English/Spanish is highly desirable.

Qualifications

• Current Illinois Physical Therapy License.
• Clinical experience desirable.
• Bilingual English and Spanish highly desirable.

Responsibilities

• Provide PT evaluation, reassessment and consultation upon referral.
• Develop plan of care, goals and discharge planning.
• Provide PT interventions within APTA guidelines.
• Participate in interdisciplinary discharge planning/continuum of care.
• Ensure patient safety, rights and regulatory compliance.
• Maintain license and CPR certification; pursue ongoing education.
