Job summary

• Bachelor’s Degree in Physical Therapy.
• Licensed Physical Therapist in State of Illinois
• BLS from American Heart Association upon hire

Responsibilities

• Demonstrates NM values in daily work and interactions.
• Maintains primary responsibility over patient plans of care with treating PT/PTA(s).
• Documents notes in electronic medical record (EMR) and completes daily progress notes.
• Provides education materials appropriate to patient understanding.
• Collaborates with healthcare teams and participates in department processes.
