Qualifications

• Bachelor’s Degree in Physical Therapy from an accredited program.
• Licensed Physical Therapist in State of Illinois.

Responsibilities

• Demonstrates the four NM values of teamwork, patients first, integrity, and excellence in daily work.
• Maintains primary responsibility over patient plans of care with timely collaboration with treating PT/PTA(s).
• Completes daily notes reflecting patient response and progress per treatment plan and discharge needs.
• Communicates effectively with patients and healthcare teams, providing clear directions and education.
• Adheres to department policies regarding evaluations, progress notes, and discharge summaries in EMR.
• Participates in committees, department processes, and special projects as per department expectations.
