Sustain Recovery Rehab Center in Long Island City, NY seeks an Outpatient Physical Therapist for a full-time, onsite role. You will deliver personalized care, evaluate patients, and create recovery plans to restore mobility and independence.

Qualifications

• Bachelor's degree in Physical Therapy from an accredit ed institution.
• Valid New York Physical Therapist license.
• Minimum of 2 years of outpatient physical therapy experience.
• Master's or Doctorate in Physical Therapy preferred.
• Experience with diverse patient populations preferred.

Responsibilities

• Conduct thorough evaluations of patients' physical abilities, limitations, and overall health to tailor treatment plans.
• Apply hands-on techniques, therapeutic exercises, and modalities to restore mobility and function.
• Educate patients on conditions and home exercise programs to support recovery.
• Monitor progress and adjust plans; keep detailed session records.
• Collaborate with a multidisciplinary team to ensure cohesive care.
• Guide patients through rehabilitation with encouragement and motivation.
• Stay updated on evolving therapeutic modalities and best practices.
