Job summary

P4 Physical Therapy seeks a dedicated Physical Therapist to provide coverage in our outpatient clinic. You will assess patients, develop treatment plans, and deliver care per established guidelines while coordinating with physicians and insurers.

Qualifications

• Graduate from a CAPTE-accredited Physical Therapist program.
• Current state license or ability to begin licensing promptly.
• CPR certification.
• Outpatient orthopedic experience preferred but not required.
• Excellent communication with patients, staff and other healthcare professionals.
• Strong multi-tasking abilities to manage the clinic effectively.
• A self-starter with a growth mindset.
• Passion for quality patient care with a compassionate approach.

Responsibilities

• Assess patients to develop and initiate treatment plans based on evaluation results.
• Review physician referrals and patient records to determine required PT treatment.
• Provide direct patient care by established protocols and guidelines.
• Document findings, progress, and instructions to patients and caregivers.
• Responsible for patient care performed by assistants and rehabilitation technicians.
• Complete a written plan of care and develop goals to attain them.
• Implement physical therapy treatment programs and essential therapies.
• Communicate with patients, caregivers, physicians, and insurance companies.
• Ensure timely documentation including daily notes, progress reports, and discharge summaries.
