Qualifications

• Requires Bachelor’s or Master’s degree; Doctorate preferred.
• Licensed Physical Therapist in the state of practice.
• Completed CEU requirements in appropriate state(s).
• Experience with EMR software preferred.
• Strong communication and teamwork skills.
• Maintains confidentiality and professional demeanor.

Responsibilities

• Assess patients and develop individualized treatment plans.
• Perform initial evaluations, daily notes, progress reports, and discharge planning.
• Document treatment and coding using EMR per guidelines.
• Collaborate with interdisciplinary team to optimize outcomes.
• Provide patient education and caregiver training.
• Oversee PTA, ATC, and aides as needed.
