Job summary

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution is required.
• Active Physical Therapist license in the state of practice.
• Graduates awaiting examination may practice under a GPT license in Rhode Island or a temporary license in Massachusetts.
• Active BLS certification.

Responsibilities

• Examines and evaluates patients.
• Establishes diagnosis, prognosis, and individualized plans of care.
• Implements treatment interventions and monitors progress.
• Directs and coordinates rehabilitation plan of care.
• Evaluates progress and revises goals and plan of care.
• Educates patients/caregivers.
• Anticipates discharge/home needs and resources.
• Completes documentation and charges accurately and timely.
• Completes clinical documentation reflecting eligibility/medical necessity.
