Job summary

BAYADA Home Health Care, Inc. is seeking an experienced Physical Therapist (PT) for outpatient and home health visits at senior living communities in Newtown Square, PA. Opportunities exist for full-time or part-time schedules across Center City Philadelphia, Haverford, and surrounding areas.

The role requires at least one year of PT experience and PA licensure, with EMR proficiency preferred. You will work as part of a multidisciplinary team to support clients staying at home and maintaining

Qualifications

• Licensed Physical Therapist in Pennsylvania.
• Minimum of one year of PT experience.
• Graduation from an APTA-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with electronic medical records (EMR) preferred.

Responsibilities

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements assessing mobility, strength, balance, and safety.
• Develop and implement individualized care plans including gait training and manual therapy.
• Reassess and revise the PT plan with nursing in the multidisciplinary team.
• Educate clients and families on rehabilitative care for independent living.
• Document observations and interventions using a patient charting system.
