Job summary

BAYADA Home Health Care is hiring a Physical Therapist for outpatient home health visits in the Newtown Square/Center City Philadelphia area. Full-time or part-time roles available; one-year clinical PT experience required.

A generous sign-on bonus of $5,000 is offered and a comprehensive benefits package is provided. Responsibilities include performing home visits, developing individualized care plans, and educating clients to promote safety and independent living.

Qualifications

• Current Pennsylvania Physical Therapist license.
• Minimum one year of recent PT experience.
• Graduation from an APTA-approved PT program (or equivalent).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem‑solving skills.
• EMR experience preferred.

Responsibilities

• Visit clients in designated geographic territories for outpatient home health care.
• Perform mobility, gait, balance, and functional assessments and measurements.
• Develop and implement individualized care plans including therapeutic exercises.
• Continuously assess and revise the PT care plan in coordination with nursing and the team.
• Educate clients and family members on rehabilitative care to promote independence.

BAYADA Home Health Care is currently seeking an experienced Physical Therapist, PT, for a Full Time or Part Time opportunity performing Outpatient H ome Health visits for our senior living office, located in Newtown Square, PA.

Full Time opportunity Monday, Wednesday, Friday at a Senior Living Community in Center City Philadelphia and Tuesday, Thursday in Haverford, PA. Part Time opportunity Monday, Wednesday and Friday in Center City, Philadelphia.

One year prior clinical experience as a licensed Physical Therapist is required. As a home care Physical Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

Responsibilities for a PT - Physical Therapist include:

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.

Qualifications for a PT - Physical Therapist include:

• A current Pennsylvania Physical Therapist - PT license.
• A minimum of one year of recent work experience as a Physical Therapist - PT.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Salary Range:

$50-55 per hour
