You will coordinate with psychiatrists and providers, assist with patient intake, authorization, and education, maintain documentation, and precept staff while supporting program marketing to clinicians and families.

Qualifications

• RN license in Illinois is required.
• ADN required for the role.
• ACLS and BLS/CPR certifications due within 90 days of hire.

Responsibilities

• Accepts reassignment as directed by management based on staffing needs.
• Assists in outpatient clinic by rooming patients and covering Nurse Desktop tasks.
• Oversees prior authorization documentation, logs for samples and buy-and-bill, and reporting.
• Conducts nurse visits to assist with medication refill decisions when patients are non‑compliant.
• Coordinates communications with referring physicians, specialists, patients, families and hospital staff.
• Documents patient history/screening responses for provider assessments with computer documentation.
• Follows Universal Precautions at all times during contact with patients and equipment.
• Monitors patients with chronic conditions and educates on next level of care to reduce readmission.
• Relays information between providers and patients, including families, pharmacies, and facilities.
• Communicates provider advice, orders and results as instructed.
• Reviews patient records for complete documentation and compliance with Core Measures and Quality Metrics.
• Serves as primary preceptor/trainer for clinical (nursing) staff.
• Supports marketing of the program with internal and external customers and PCPs.
