Job summary

BAS HealthCare is seeking a full-time Family Nurse Practitioner (FNP) or Physician Assistant (PA) to join its hybrid primary care team in Wethersfield, Connecticut. The role involves providing remote care and supporting clinical teams at various locations while ensuring excellent patient services.

The ideal candidate will have over 5 years of experience in primary care, possess a valid Connecticut Medical License, and be committed to quality care in a dynamic health-centered environment.

Qualifications

• Experience in a primary care setting as an FNP or PA is required.
• Knowledge of virtual care services and protocols is necessary.
• Ability to work flexible hours supporting a hybrid schedule.

Responsibilities

• Join a hybrid remote primary care team.
• Provide remotely scheduled Transition of Care and Wellness Visits.
• Support in-person patient care at Campus Care locations.
