Proforma Health Inc. is seeking a Physical Therapist Assistant for an outpatient rehabilitation setting, delivering compassionate, hands-on patient care under supervision by a licensed Physical Therapist.

You will assist with treatment plans, educate patients on exercises and safety, and support therapeutic interventions while maintaining accurate records and collaborating with a multidisciplinary team to ensure patient-first care.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current licensure/registration as required by state regulations.
• Strong communication and interpersonal skills with a compassionate approach.
• Ability to follow clinical direction and apply evidence-based practices.
• Comfort working in a team environment and supporting patient education.
