Affinity Home Health Care Inc is seeking a Credentialed Physical Therapist Assistant to provide evidence-based outpatient rehabilitation under a licensed Physical Therapist. You will assist with individualized treatment plans, guide therapeutic exercises, and monitor progress to improve function and independence.

You will collaborate with a multidisciplinary team, educate patients and families on exercises and home routines, and maintain accurate documentation to ensure safe, high-quality care.

Formación

• Credentialed Physical Therapist Assistant with knowledge of rehab principles.
• Strong patient communication and empathy.

Provide compassionate, evidence-based support in an outpatient rehabilitation setting under the direction of a licensed Physical Therapist. Assist with implementing individualized treatment plans, guiding therapeutic exercises, and monitoring patient progress to improve function, mobility, and independence.
