Job summary

Coastal Home Health & Hospice is seeking a Physical Therapist for our outpatient rehabilitation clinic in Brookings, OR. You will restore function, improve mobility and reduce pain under physician oversight.

Responsibilities include evaluating patients, creating plans of care, delivering therapeutic interventions, educating families, and coordinating with the care team while maintaining documentation and compliance with regulations.

Qualifications

• Graduate of an accredited program in Physical Therapy
• Licensed by the Oregon Physical Therapy Licensing Board
• Current BLS certification (or obtain prior to start date)
• One year of outpatient orthopedics or related rehab preferred; new grads considered with strong experience

Responsibilities

• Evaluates patients referred for outpatient PT services
• Develops outpatient plans of care with goals and timelines
• Administers therapeutic exercises, manual therapy, modalities, gait training
• Educates patients and families on exercise programs and injury prevention
• Documents evaluations, treatment notes, progress and discharge summaries
• Collaborates with referring providers and clinic staff to coordinate care
• Adjusts treatment plans based on progress and reassessment
• Supervises PT assistants in accordance with state acts and policies
• Maintains knowledge through continuing education
• Ensures compliance with guidelines and infection control
