Qualifications

• Bachelor’s Degree in Physical Therapy.
• Licensed Physical Therapist in State of Illinois
• BLS from American Heart Association upon hire

Responsibilities

• Direct patient care and teaching in outpatient clinic.
• Participation on appropriate committees and departmental process improvement.
• Maintains patient plan of care with treating PT/PTA(s) and documents in EMR.
