is currently seeking an experienced Physical Therapist, PT for a full-time outpatient home health visits position at our senior living office in Exton, PA. This office services adult and geriatric clients throughout the West Chester area.

Responsibilities:

1. Make outpatient visits to clients in designated territories.
2. Perform diagnostic tests and measurements such as mobility, joint range, transfer status, stability, ambulation patterns, muscle strength, balance, and safety assessments.
3. Develop and implement individualized care plans including manual exercises, gait training, and balance interventions.
4. Assess and revise care plans regularly, collaborating with nursing staff as needed.
5. Educate clients and their families on rehabilitative activities to promote health, safety, and independence.
6. Document observations and interventions accurately using electronic medical records.

Qualifications:

• Current PA Physical Therapist license.
• At least one year of recent PT experience.
• Graduation from an accredited program recognized by the American Physical Therapy Association.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; EMR experience preferred.
