Qualifications

Responsibilities

• Develop and adjust treatment plans considering discharge needs and patient progress.
• Conduct assessments, reassessments, and consultations.
• Educate patients and collaborate with healthcare teams.
• Delegate tasks appropriately, ensuring safety and competency.
• Complete accurate documentation per policies.
