Concord Hospital is seeking a Physical Therapist I for outpatient services to evaluate patients across ages and populations. You will develop and implement treatment plans in accordance with licensure and departmental standards, and re-evaluate progress over time.

You’ll document to meet HIMS/QA standards, maintain productivity for patient access, and manage insurance and billing. Collaboration with a multidisciplinary team is essential for optimal outcomes.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Current NH physical therapy license required.
• Current American Heart Association BLS for Healthcare Providers or equivalent.
• Experience working with a multidisciplinary team and diverse populations.

Responsabilités

• Performs patient evaluations and develops treatment plans aligned with licensure and department standards.
• Re-evaluates patients according to department standards and updates goals for episodes of care.
• Documents in compliance with HIMS/QA standards and meets medico-legal requirements.
• Maintains productivity to support patient access and clinic needs.
• Manages insurance and charging requirements and participates in team development.
