Job summary

Connectly Recruiting in Gratiot, OH invites a licensed Physical Therapist to join our outpatient rehab team at a Forbes-listed hospital. You will evaluate and treat patients, work across settings during clinical rotations, and use EPIC EMR for documentation.

Positions offer 31, 33, or 40 hours per week with a sign-on bonus and full benefits. Ohio license required or eligibility; prior rehab rotations preferred.

Qualifications

• Degree in Physical Therapy required.
• Current Ohio Physical Therapy License or able to obtain.
• Minimum of one clinical rotation in a Rehabilitation Services setting (e.g., Acute Rehab, SNF, Acute Medical, Outpatient, Home Health, and/or Behavioral Health).

Responsibilities

• Evaluate, plan, and provide treatment for patients referred to the service.
• Deliver group and/or individual treatment to patients with a variety of diagnoses and presenting symptoms.
