OSR Physical Therapy is a private, therapist-owned practice established in 2005 with six locations in the SW Metro. We provide outpatient PT services including sports/orthopedics, vestibular/concussion, pelvic health, pediatrics, athletic training outreach, and wellness plans.

We are seeking a dedicated Physical Therapist to join our healthcare team to deliver comprehensive PT care and collaborate with a multidisciplinary team for optimal patient outcomes.

Qualifications

• Graduate degree from a CAPTE-accredited Physical Therapy program.
• Current Minnesota state physical therapy license or eligibility to obtain one.
• Valid CPR certification.
• Strong knowledge of anatomy, physiology, and rehabilitation principles.
• Excellent assessment and diagnostic skills.
• Proven ability to develop and implement effective treatment plans.
• Strong documentation and electronic health record skills.
• Outstanding interpersonal and communication abilities.

Responsibilities

• Conduct thorough patient evaluations and develop individualized treatment plans.
• Implement evidence-based physical therapy interventions and monitor patient progress.
• Document patient assessments, treatment plans, and progress notes with attention to detail.
• Supervise and coordinate care delivered by physical therapy assistants and rehabilitation technicians.
• Establish measurable treatment goals and adjust plans based on patient progress.
• Provide patient and caregiver education regarding treatment plans and home exercise programs.
• Collaborate with referring physicians and other healthcare providers.
• Maintain accurate and timely documentation in compliance with healthcare standards.
• Utilize rehabilitation equipment and techniques effectively.
• Participate in regular team meetings and care coordination discussions.
