Job summary

Northwestern Memorial Hospital in Chicago is seeking an experienced physical therapist to join our in-house registry team at the Outpatient Old Irving Park Rehab Clinic. This hourly position requires a minimum commitment of 16 hours per month, with clinic hours Monday through Thursday 7:00 AM–7:00 PM and Friday 7:00 AM–5:00 PM.

The ideal candidate has outpatient experience, Illinois licensure, and BLS certification.

Qualifications

• Bachelor’s Degree in Physical Therapy required.
• Licensed Physical Therapist in State of Illinois required.
• BLS from American Heart Association upon hire required.
• Masters or Doctoral Degree in Physical Therapy preferred.

Responsibilities

• Demonstrates NM values in daily work and interactions.
• Maintains primary responsibility over patient plan of care in collaboration with PT/PTA(s).
• Completes daily notes reflecting patient response to treatment and status for discharge.
• Maintains documentation in EMR and adheres to departmental policies.
