Qualifications

• Bachelor's degree in Physical Therapy is required.
• State license to practice Physical Therapy is required.
• BLS certification per policy guidelines is required.

Responsibilities

• Evaluates patients and develops care plans with modalities, procedures, and exercise programs.
• Collects data from records and patients to inform treatment.
• Sets functional goals and provides evidenced-based interventions.
• Documents evaluations, tests, progress notes, and discharge information.
• Communicates progress to physicians and care team; coordinates treatment and discharge planning.
• Supervises PTA's and Rehab Tech/Aide as required.
