Job summary

Transformational Health Network, Inc. is seeking a compassionate Physical Therapist to join its outpatient rehabilitation team in Ithaca, NY.

You will evaluate patients, develop individualized treatment plans, and deliver evidence-based interventions in a collaborative, outpatient setting. Qualifications

• DPT or equivalent from an accredited program.
• Current PT license or eligibility to practice.
• Outpatient PT experience preferred.
• BLS certification (or ability to obtain).
• Excellent communication, organizational, and interpersonal skills.
• Ability to treat orthopedic, neurological, and musculoskeletal conditions.

Responsibilities

• Perform comprehensive patient evaluations and develop individualized treatment plans.
• Deliver evidence-based physical therapy interventions including exercise, manual therapy, modalities, and functional training.
• Educate patients on home exercise programs, injury prevention, posture, and long-term wellness.
• Monitor progress and modify treatment plans to achieve rehabilitation goals.
• Collaborate with physicians and interdisciplinary teams to coordinate care.
• Maintain accurate clinical documentation in accordance with standards.
• Promote a safe, organized, and patient-focused treatment environment.
