Qualifications

• Bachelor's Degree in Physical Therapy.
• Licensed Physical Therapist in Illinois.
• BLS from the American Heart Association within 90 days of hire.
• Masters or Doctoral Degree in Physical Therapy preferred.
• Lymphedema Certification preferred.

Responsibilities

• Oversee patient plan of care through collaboration with treating PT/PTA(s).
• Maintain EMR documentation with evaluations, progress notes, and discharge summaries.
• Provide timely updates and education to patients and families.
• Collaborate effectively with healthcare teams and other departments.
• Contribute to continuous improvement and cost-reduction initiatives.
• Flex schedule to meet patient care needs and department meetings.
