Beacon Harbor Healthcare in Rockwall, TX is seeking an Outpatient Physical Therapist to join their in-house therapy team. This full-time role offers PRN schedule opportunities and focuses on ambulatory therapy for geriatric and post-acute patients.

You will conduct screenings, develop care plans, administer rehabilitative treatments, and provide discharge summaries in compliance with regulatory standards. A Physical Therapist license is required; new grads welcome and all experience levels

Responsibilities

• Deliver rehabilitative, skilled, and medically necessary treatments.

• Provide counseling to patients, families, and other providers regarding physical disorders.
• Generate comprehensive discharge summaries in compliance with regulatory and clinical requirements.

Beacon Harbor Healthcare in Rockwall, TX, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a Full Time Physical Therapist to assist with our outpatient ALF programs. Consistent PRN hours are also available.

Key Responsibilities:

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications:

Physical Therapist license is required.
