Job summary

UChicago Medicine seeks a Pharmacist- Outpatient, Burr Ridge, to join the population health team. You will optimize medication use, support quality metrics, and drive cost-effective pharmaceutical care across our healthcare system.

This role emphasizes data-driven review of high-risk patients, MTM/CMM services, and collaboration with physicians and nurses to close gaps in care while upholding patient safety and evidence-based practice.

Qualifications

• Graduate of an ACPE accredited School of Pharmacy.
• Doctor of Pharmacy or another advanced degree preferred.
• Registered pharmacists in the state of Illinois or eligible for same.
• Exceptional aptitude or knowledge may be considered in lieu of experience.
• Knowledge of contemporary hospital/clinical practice and service.
• Strong written and verbal communication skills.
• Ability to interact constructively with supervisors, peers, support personnel, and students.
• Knowledge and understanding of general hospital operations.
• Ability to integrate clinical services, distributive services, education, and research activities.

Responsibilities

• Medication Management and Optimization: conduct comprehensive medication management (CMM) and MTM services for high-risk patients with complex chronic conditions.
• Optimize medication regimens, manage polypharmacy, and resolve drug-related problems, including adverse events, non-adherence, and cost barriers.
• Perform medication reconciliation during transitions of care to prevent errors and reduce hospital readmissions.
