Job summary

St. Luke’s Health System in Meridian, Idaho is seeking a Full-time Outpatient Pharmacist to join our Outpatient Pharmacy.

This role plays a key role in delivering high-quality pharmaceutical care across multiple outpatient service areas, including medication lockers, traditional outpatient pharmacy services. The ideal candidate will be comfortable with the clinical aspects of pharmacy practice, thrive in a collaborative healthcare environment, and be motivated to support innovative outpatient

Qualifications

• PharmD or equivalent from an accredited school.
• Pharmacist license in Idaho or eligible for licensure.
• BLS certification required.

Responsibilities

• Review and evaluate prescribed therapies for appropriateness.
• Interpret prescriptions; compound and dispense medications.
• Counsel patients, nurses, and staff on medications.
• Perform clinical pharmacy services and document interventions.
• Administer medications and vaccines as appropriate.
• Supervise and mentor pharmacy staff and technicians.
• Maintain medication records, inventory, and charges.
• Support transitions of care and Meds-to-Beds initiatives.
• Provide training to technicians and residents.
