Job summary

Intermountain Health St. Mary's Regional Hospital in Grand Junction, Colorado, is launching a new outpatient pharmacy and seeks an Outpatient Clinical Pharmacist to help build workflows, optimize patient care, and oversee daily operations with the Pharmacy Manager and team.

This PRN role requires a Doctor of Pharmacy, active licensure, and knowledge of state/federal regulations; a collaborative, proactive pharmacist who can deliver safe, cost-effective care and engage with patients.

Qualifications

Doctor of Pharmacy or Bachelor of Science in Pharmacy degree from an accredited institution.
License to practice pharmacy in the state of practice and dispense controlled substances.
Current knowledge of state and federal regulations and statutes regarding the practice of pharmacy in the state of practice.
Effective communication, people management, and project management skills.
Immunization certification if applicable to practice site needs

Responsibilities

Reviews patients' medication orders for allergy interactions, duplication of therapy, appropriateness of drug therapy, and drug-drug interactions; makes interventions to optimize medication use.
Oversees and/or ensures timely preparation and delivery of medications to care areas; ensures secure storage and workflow.
Provides counseling to patients and promotes medication education and adherence.
Supervises/instructs interns, technicians, and staff; provides medication-related clinical consultation to health professionals.
Solves problems related to equipment, maintenance, drug availability, staffing, and patient care concerns.
Identifies process improvements to enhance patient satisfaction and safety; participates in departmental meetings.
