Job summary

Qualifications

• Bachelor's degree in Physical Therapy required.
• Applicable state license for Physical Therapy required.
• One year of experience as a licensed Physical Therapist.
• Basic Life Support for Healthcare Provider certification required.
• Meets Rehab Services Career Ladder Guidelines for a Level II therapist as approved by the Rehab Credential Council.

Responsibilities

• Evaluates patients and establishes a patient care plan with modalities, procedures, exercise programs, therapeutic groups and patient education as warranted by the diagnosis, age and discharge needs.
• Gathers and interprets data from the medical record, physician reports, patient and family.
• Establishes functional goals and provides evidence-based intervention with ongoing assessment to adjust the plan of care as needed.
• Documents evaluations, tests, progress notes, treatment notes, attendance, education, outcomes and discharge notes.
• Communicates evaluation and progress to physicians, referral sources and other clinical staff; coordinates schedule and discharge planning.
• Provides coverage at other departments following orientation and competency for the assigned level of care.

Education

Bachelor's degree in Physical Therapy program

Applicable state license for Physical Therapy
