Responsibilities

• Screen and evaluate patients with physical functioning disorders and develop appropriate care plans.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions.
• Involve regular standing and walking; assist with transfers and positioning as needed.
• Administer procedures as part of the rehabilitation plan, including manual techniques, exercises, modalities and wound care.
• Provide counseling to patients, families, and caregivers regarding physical disorders.
• Generate discharge summaries ensuring regulatory and clinical compliance.
