Atrium Health is seeking a licensed Physical Therapist to evaluate patients, develop and implement comprehensive care plans, and guide exercise programs based on a holistic view of diagnoses and discharge needs. You will gather data from medical records and collaborate with physicians and clinical staff, set functional goals with patients, document progress, and coordinate scheduling and discharge planning.

Supervision of PTA and rehab Tech staff is also required.

Responsabilidades

• Evaluates patients and establishes individualized care plans using assessment techniques and evidence-based modalities.
• Gathers and interprets data from medical records, physicians, patients and families; documents findings.
• Sets functional goals with the patient and provides evidence-based interventions with ongoing plan adjustments.
• Documents evaluations, progress notes, discharge notes, education, and related charges.
• Communicates patient progress to physicians and staff; coordinates schedules and discharge planning.
• Supervises PTA and rehab technicians as required for licensure.
